Pyloric Stenosis (Infantile Hypertrophic Pyloric Stenosis)

What is Pyloric Stenosis?

Pyloric stenosis, also termed infantile hypertrophic pyloric stenosis (IHPS), is a condition wherein there is abnormal thickening of the muscle at the outlet of the stomach. This narrowing blocks milk/food from passing easily into the small intestine and creates feeding problems for infants.

Signs and Symptoms

These infants are often healthy at birth but develop severe, powerful vomiting within weeks usually at 2 to 8 weeks of age. Vomiting gradually becomes projectile and the baby vomits milk, never bile. Babies are hungry despite vomiting and therefore want frequent feeds. Other signs include dehydration, less wet nappies (decreased urine output), and visible stomach contractions.

Why is early detection important?

Projectile vomiting can quickly cause dehydration and an imbalance of important electrolytes, such as sodium, potassium and chloride. Without early treatment, serious complications may develop that can affect an infant’s growth and overall health.

How is pyloric stenosis diagnosed?

It is diagnosed by ultrasound abdomen which shows thickened pyloric muscle. Also, a good history and clinical examination is of paramount importance in suspecting this diagnosis. 

Management of IHPS

The first step is stabilizing the baby by correcting dehydration and electrolyte imbalance with IV fluids. Surgery, called pyloromyotomy, is the definitive treatment. This surgery is safe in experienced hands and is usually performed with a small abdominal incision. 

Recovery and Outlook

Feeding can commence within hours after surgery, with a gradual increase in volume of feeds. Occasional vomiting may occur post-operatively but is usually temporary. Long-term results are very good, with almost all babies growing normally.

Any infant with repeated, forceful vomiting, signs of dehydration, poor weight gain, or lethargy should have urgent medical evaluation. Early diagnosis and treatment prevent complications and ensure normal growth.
